For your convenience, we use online new patient registration.
Here is how the process works:
- Once your appointment is made, we will generate a username and password which we will send to you via text message or email. This information will allow you to log in to the patient portal and complete all the forms and paperwork necessary for the new patient registration.
- It’s easy and efficient to complete all online patient registration forms (Medical History, Dental & Pain History, Insurance information, etc) at the comfort of your home vs at our office.
- If you do not have access to the computer, we are happy to help you at our office to complete all the necessary forms required for the patient registration. Please arrive 20 minutes prior to your appointment to allow time to complete these forms.
